Peer Advocacy and Support services help people get started on and continue their own positive pathways to behavioral health and wellness. Our peers serve as educators, advocates, supports and connectors to other resources.
Supportive Housing programs allow people to live anywhere in the community, receive help with their rent through subsidies, and receive customized services from staff to ensure they are living well and meeting their self-defined goals.
Our Care Coordinators work closely with Medicaid Health Home enrollees to develop and implement person-centered plans of care. We help coordinate all chosen services, supports, and resources into a “Wellness Team.”
CORE (Community Oriented Recovery and Empowerment) Services provide opportunities for eligible adult Medicaid beneficiaries with mental and/or substance use disorders to receive services in their own home or community.
