People USA is proud to be on the front lines of de-institutionalization and the drive towards full community integration for all. To that end, we work with people currently living at the state-run Rockland Psychiatric Center to help them transition back to community living, so that they can live the life they envision.
The Mobile Outreach Team is made up of a highly trained People USA peer, a social worker, and a nurse. Our combined expertise is used to provide transitioning residents with hands-on exposure to various dimensions of life and wellness, and to help them focus on and implement their own goals. This can include helping develop skills and access resources needed to increase their capacity to be successful and satisfied in the living, working, learning, and social environments of their choice.
“I was actually in the hospital, I was in the hospital one other time before that for mental health problems. At that time, I was homeless and I needed help. Zach, my peer advocate came. And he sat me down and we started talking and he pointed out all the great things about this program. I was interested right away and it’s been very helpful to me ever since. He’s also been very intuitive in helping me with which other peers I should be talking to about housing and benefits and also the job situation.” – Michael D.
