What is transition of care?

Transition of Care Specialist provides participants with the opportunity to receive community-based case management services regardless of their ability to pay.

We specialize in facilitating communication between medical professionals, mental health professionals, patients, and their families by creating a safe and trusting environment for the people we work with.

We help participants transition from one stage of their lives to another, while providing participants with a greater knowledge of local resources that exist.

Qualified Staff

Our specialists are New York State Certified peers and have personal experience with mental health and/or substance abuse issues.

 

Person-centered

Our specialists use a person-centered approach. They meet people where they are to assist them in taking the steps needed to accomplish goals and live a happy, healthy, and independent life.

 

Resource Experts

Our specialists provide participants with a greater knowledge of the local resources that exist in Warren/Washington County, and other surrounding local counties.
